Was wondering if anyone out there,knew of any websites or
books,magazines etc that have forms or questionnaires that
are available for nightclub surveys for the public.

dont know but, i would just go to the other similar clubs in the area (the most direct competition) and see how busy they are.
maybe casually ask the bartender or the doorman etc how well the club does in that neighborhood, what days are slow, are there a lot of fights(that is a liability for you if it happens at your club), how hard was it to get a liquor license in that area(you'll have to contact your city planning department to see if they're even willing to have a club with a liquor license in the area you're looking at. things like nearby residence and schools can be roadblocks just off the top of my head)
I wouldnt come off too nosy though, just make small talk and try to gain rapport with them. i dont know if i would tell them im trying to open a club nearby though they might not want to give you any information then heheh
What are you trying to learn? Who is your target audience?
I've done some survey design (I was VP of Engineering for an online survey company several years back).

Thanks for your replies.Here in Montreal,bar owners and their employees don't give any info if
they don't know you.Most nightclubs are geared to
the young popmusic fans etc.The research i've
been doing for the past 6-8months is for the
country music enthusiasts thus more "blue" collar
than "white" collar as well as a more mature bunch
of patrons.Need a general "entertainement" survey
that i can have distributed to approx 1,000-2,000
homes(ranchers,farmers,etc)in and a radius of 30
miles to where i want to open it.Sort of like....
..how often you go to bars?,what age group?,what
type of entertainement?what kind of beverage?etc.,
just to give an idea.
