I am trying to write a code of ethics/ code of conduct. Any advise would be helpful.

Do you mean a code of conduct for your customers? Or a code of ethics for bars in general?
I've worked with other bars on a customer code of conduct, and it ended up being a pretty bar specific project (i.e. the bars with bands didn't get the point of 'no gambling', and the sports bars didn't get the point of 'no moshing'). Still waiting to find that punk rock sports bar that needed both rules for their customers.

The Tavern League of Wisconsin has a good code of ethics for it's members:

The Chicago Hospitality Association adapted there own code based on Illinois specific liquor policies (i.e. the crazy no happy hour law) but might not apply to your area:

Most of the codes I've found stay pretty general. Save the details for the employee handbook.
As far as customer code of conduct/ethics I think it's so highly dependent on your crowd and venue that a good model is hard to find - a "no moshing" rule makes no sense for a sports bar, and a "no gambling" rule doesn't make much sense for a rock club.
Whatever you come up with, don't skip the forehead smacking obvious rules. There is always "that guy" who needs acceptable behavior spelled out for him - usually in short words a five year old can understand.
